After a few parents asked me about pricing for Liberty Rec sports, I've decide to post this registration form. This is the current form being distributed as of February 1, 2012*.

Note: $5 outside city limit charge. Sometimes, depending on the team sponsorship; shirts are included. Pants, shoes, gloves are not normally included.
